How to Take history of the client

For a healthy counseling session, you must need to take history of the client so that you can do the counseling session. For taking the history, you can follow below given format.

·        Name of the client

·        Age

·        Sex

·        Address

·        Contact Number

·        Education Qualification

·        Occupation

·        Monthly Income

·        Marital Status

·        Name of the parents

·        Name of the spouse

·        Total family members in the home

And according to your client profile, you can add more questions. Like if you are taking history of a drug user, then you can add following points

·        Types of drugs you are using

·        Age of initiation of drug

·        Last date when you used drugs

·        Quantity of the drugs

·        Withdrawal Symptoms

·        Impacts on health

·        Impacts on family
